Health Risk Assessment 2017 Registration

Registration for the COS Employee Health Risk Assessments will be open February 20, 2017.

Registration is fully online and easier than ever! Follow these easy step-by-step directions to help you sign up through Mercy BioCheck.

Registration Directions

  1. Follow this link to get started (Open in Google Chrome): http://www.wellnesspartner.org/
  2. Click on "Events" in the top right
  3. Scroll over to the month of April or Mat to find the dates/locations of the HRAs
  4. Be sure that you are selecting the HRAs specific to COS (City of Springfield)
  5. Fill out the required information and click "Next"
  6. You can add this event to your Outlook Calendar - however, you have to manually change the time of your apppointment
  7. You will receive a confirmation email with appointment details
  8. You will also receive another confirmation email 2 days prior to appointment
